Please arrive fifteen minutes early for your initial visit.

Please bring:

  • New patient or procedure paperwork
  • Identification
  • Medicare card (if applicable)
  • List of prescription medications
  • Other information our doctor may need to make an accurate evaluation of your health (x-ray reports, lab results, any additional concerns you may have, etc.).
  • A photo of the front and back of any supplements and over-the-counter medications you take is very useful. (See example.)

After your initial visit it is only necessary to bring updated information on prescriptions, tests, or procedures another doctor may have ordered.



Alabama Functional Medicine offers a Membership Program for those patients who need continuing care from our physician. A member’s monthly fee covers increased access to a physician to spend time on prevention or cure and to discuss helpful technologies with particular focus on the latest in regenerative treatments that may or may not be covered by insurance. This access includes text, home phone, email, and a very dedicated office staff and lab specialist who are very motivated to help you get the most out of your membership.

Alabama Functional Medicine provides in-office lab services through Cleveland Heart Lab®. We chose this lab due to their extensive testing options with inflammation, autoimmune diseases, and cardiovascular disease. These frequently help us to drill down to the exact cause of chronic illnesses. You will find the lab tech Nikki is one of the most pleasant and efficient technicians you have encountered.

Cleveland Heart Lab® is in network with essentially all insurance carriers. For your convenience, the lab will file test claims directly with your insurance company. Please contact your insurance carrier for details on your specific coverage. All billing is handled through the lab. For billing questions please contact the lab.


Our lab is open Monday through Thursday from 7:45 a.m. to 3:00 p.m. with no appointment required.


Alabama Functional Medicine does not accept or bill insurance for our services, with the exception of Medicare. Some of the advanced procedures are not yet covered by Medicare insurance. Please consult your carrier. You can still use healthcare insurance to cover things such as labs, prescriptions, imaging, and for any other medical providers you may see. Membership with our practice is not a substitute for insurance.

Due to the rising cost of healthcare insurance for those patients in need of an alternative healthcare coverage option we suggest Liberty Direct, a medical cost-sharing program that Dr. McWhorter and his family have chosen for their own medical coverage. Since Dr. McWhorter is a Liberty Direct providing physician you may be eligible for monthly reimbursements to subsidize Dr. McWhorter’s membership cost. The program is administered by Liberty HealthShare, a national leader in healthcare cost-sharing. Liberty HealthShare is a community in which each person is committed to help their fellow members as they face unexpected medical costs and contend with hyper-inflated prices resulting from the third-party payer medical system.
